From the breeder or shelter
The first DHPP dose or two, sometimes a deworming record, and the microchip number if one was implanted. Get the actual dates and product names, not "had first shots".
8 to 16 weeks: the vaccine series
- DHPP every 2 to 4 weeks until at least 16 weeks old. Each visit is a certificate or an entry on the clinic record.
- Rabies at 12 to 16 weeks, depending on state rules and the vet. This is the first rabies certificate, valid for one year.
- Bordetella, leptospirosis and canine influenza as your vet advises, based on what the puppy will do.
Microchip
Often done at a vaccine visit or at neutering. The number is only useful if it is registered to you; see the microchip checklist.
License
Most counties and cities require a dog license once the rabies vaccination is done, and they want the certificate to issue it. Keep the license with the record.
Spay or neuter
Timing is a conversation with your vet. The surgery record and discharge notes are a vet visit entry; kennels and landlords sometimes ask about status.
Around one year: the first boosters
DHPP booster (many vets now give it at 6 months), rabies booster (this one may be a 3-year product), and annual repeats of any lifestyle vaccines. From here on the intervals stretch out, which is exactly when reminders start earning their keep.
